Apply for a Library Card

You can apply for a free library card from E. D. Locke Public Library by filling out the online form or visiting the library in McFarland, WI. Anyone who lives in the South Central Library System service area can apply for a free library card valid at all 65 libraries in the system. If you already have a card from another member library, there is no need for a new card. 

To apply:

  • Apply on-line
  • Request an application form at the library or complete at home: Library Card Application Form
  • Once the on-line form is submitted, you will receive an email indicating the form has been received.  Please stop by the library after 2-3 days to pick up your new library card.  (Please bring photo ID with you.)
    • Acceptable proof of address: utility bill, car registration, bank statement, check, renter's agreement, closing documents, etc.
  • Children under 16 must be accompanied by a parent or legal guardian and come to the library in person to sign the registration form and present a photo ID and proof of address.

Lost/expired cards or forgotten PIN: Please call the library at 608-838-9030 or visit the main desk for assistance.

Address changes: Please bring in photo ID and proof of address if your ID has not been updated accordingly (see examples above).
